Illness/injury/surgery: Please call the nursing office as soon as you are aware of an injury, illness or upcoming surgery. We ask this to assist in planning your student’s needs. If a student has had surgery or injury, please obtain a medical note from Physician indicating the student’s restrictions and needs. (EX. “No gym until cleared by MD", "Needs to use elevator due to injury”) If a student will require administration of medications during the school day that cannot be given at home, please complete the School Medication Authorization Form. Please remember that students cannot bring in their own medications; a parent must deliver medications and the School Medication Authorization form to the nursing office. If you have any questions regarding this procedure please call 870-6838.

Absence from School: 870-6838 - Please call the nursing office daily if your child is absent. Please indicate reason for absence. If a student is absent or misses part of a school day for a medical reason and has seen a Physician, we encourage you to obtain an MD note at the time of the visit.

Mandatory 10th grade physical: “an immunization update and additional health assessment are required in the 6th and 10th grade” as specified by Tolland BOE and Connecticut General Statutes Sec 10-204a and 10-206. These physical exams must be completed on the Health Assessment Record form that is provided by the state. Physical exams done up to one year prior to the student entering 10th grade are acceptable. A form will be sent home with all 10th graders at the start of the 10th grade. Reminder letters will be sent home to parents of students who have not turned in their physical form by January of the 10th grade. Sports Physicals: The Connecticut State Medical Society’s Committee on the Medical Aspects of Sports has required a Pre-Participation sports examination yearly for all students who wish to try out and participate in sports activities. Here is the Sports Physical form and a link to the THS athletic site. *Please note if you are entering 10th grade, you will need both the Health Assessment Record and the Pre-participation Sports Physical form filled out by your physician.*

Medications: Prescription and over the counter medications must be accompanied by a Physician signed “Authorization for the Administration of Medicine by School Personnel” per Connecticut State Law and regulations 10-21(a). Medications must be delivered to the School nurse by the parent properly contained with a label affixed bearing the patient’s name, physician, medication dose directions for administration and prescription number. Medications not picked up by the end of the school year will be discarded. Please complete the School Medication Authorization form.

Breakfast? Please be aware that we encourage all students to have a healthy breakfast. Many students are rushed to get to the bus and school and don’t have time to eat. We see many students for complaints of headaches and stomach aches most time related to not having eaten. Please encourage them at home to prepare something the night before or in the morning for a grab and go meal to get them started. Thanks…If you have any questions, please feel free to contact us.
